Facility Overview

Feel right at home at the Essentia Health-Living Center. Our long-term care center in Fosston, Minnesota, offers 24-hour skilled nursing care. We care for people long-term and short-term, and offer adult day services. While providing the best care possible, we respect each resident’s personal wishes. Many of our staff members have been with us for a long time and truly enjoy getting to know our residents. Many are also trained in Alzheimer's and dementia care, hospice, infection control, and antibiotic stewardship.We’re attached to the Essentia Health-Fosston hospital and clinic, so you can get a wide range of medical care in one location. On site, we offer rehabilitation nursing and our own bus for fun outings. Our activities department also hosts games, music, and more so you can connect with other residents.

Onsite Services

Get a wide range of services within our long-term care center, including:

• 24-hour skilled nursing care
• Adult day services – Care for adults between 8 am to 4:30 pm, whether it’s one day a week or every day of the week
• Transitional care – Temporary care when hospital-level care isn’t needed but patients aren’t ready to go home, like after a surgery or accident
• Respite care – Temporary care when normal caregiver isn’t available
• Hospice care – Compassionate end-of-life care along with skilled nursing care
• Physical, occupational, and speech-language therapy
• Restorative nursing
• Social services
• Chaplain services – Spiritual care
• Wound care
• Doctor visits monthly

History

Our doors opened in the 1960s as a 50-bed skilled nursing home connected to the local hospital. Since then, Essentia Health Living Center has grown in quality, compassion, and comfort. By listening to our residents, we’ve moved from an institutional model of care to one that focuses on each resident’s needs and wishes. In 2016, we updated our flooring, paint, wall decor, and furniture to make our center even more inviting.
